Required Forms: 2025-2026 School Year

At the beginning of each school year, parent/guardians are asked to complete the following forms for the 2025-2026 school year by clicking the links below. The electronic forms are closed two weeks after school starts. Students starting school at a later time will be provided these forms by the main office of your child's school.
  
Health Information and Consent Form - Complete by Thursday, August 28

• Complete by Friday, September 5

• Acknowledgement Form for Parent/Student Handbook; Code of Student Conduct; and Board Policy Manual
• WSD Publicity Opt Out Form
• HSA/WEOF Communication Opt Out Form
• Military Opt Out Form (11th & 12th grade only)

• Complete the form for the school your child attends:

• Acceptable Use (WSD Computer/Internet) Form- Elementary
• Acceptable Use – WSD Computer/Internet Form – Middle School
• Acceptable Use – WSD Computer/Internet Form – High School
